About Us:
Main Street Health is the nation's largest provider of value-based care focused exclusively on serving rural America. We believe in the old ways of medicine, when healthcare was simpler. By partnering with local providers, we reinforce trust and relationships at the heart of care in rural communities.
Rural providers also need more resources to ensure patients get the right care at the right time. That's why we give our partner clinics the staff, technology, and opportunities they need to succeed in a value-based delivery model.
About the Role:
The Project Director is a senior individual contributor who owns the end-to-end implementation of the Main Street clinical model as we expand into new practices, new markets, and new products. You will personally lead multiple concurrent implementations from kickoff through go-live, driving results through influence across a matrixed organization.
This is a hands-on role. You will get to build the plans, run the meetings, write the documentation, and resolve the issues. You will coordinate the clinical, operations, technology, data, and partner teams who contribute to each launch.
Responsibilities:
Lead partner implementations
- Own the successful deployment of new partner implementations and other growth initiatives across multiple markets at once
- Coordinate cross-functional teams (clinical, operations, technology, data, and functional leads) to launch new services in each market
- Track each partner's scope of services, contractual commitments, and operational nuances so nothing is missed at handoff
- Identify technology, data, and operational issues early, get them prioritized with the right owners, and drive them to resolution
- Ensure implementation processes are effective, compliant, and consistent with Main Street Health standards
Plan and execute
- Build and maintain detailed project plans from kickoff through go-live and stabilization
- Break work into clear, sequenced tasks with owners and due dates, and hold contributors accountable to them
- Plan and facilitate project meetings, keeping stakeholders aligned on status, decisions, and next steps
- Proactively surface risks and dependencies, recommend solutions, and escalate when needed
- Run post-implementation reviews to capture lessons learned and improve the playbook for future launches
- Produce and own the quality of standard deliverables, including status reports, agendas, meeting notes, and launch checklists
Qualifications:
- 3 to 5 years of project management experience, with a proven track record of delivering projects on time and in scope
- Experience independently leading multiple concurrent projects and coordinating many teams and stakeholders
- A demonstrated ability to get results through influence in a matrixed organization, without direct authority
- Strong analytical, quantitative, and problem-solving skills, and a willingness to roll up your sleeves
- Excellent written and verbal communication, including turning complex information into clear documentation
- Exceptional organization, time management, and attention to detail
- Proficiency in Excel and PowerPoint
- Flexibility, resourcefulness, and composure under pressure in a fast-paced, high-growth environment where change is the norm
Preferred:
- Healthcare experience, ideally in value-based care, primary care, or provider operations
- Experience implementing clinical programs or onboarding provider partners
- Hands-on experience with Smartsheet, Rocketlane, or similar project management tools
- PMP or similar project management certification
Role Details:
- Type: Full-time, individual contributor (no direct reports)
- Reports to: SVP of Program Delivery
- Location: Hybrid, based in our Nashville office
- Travel: Occasional travel to partner clinics and markets may be required
Those who thrive at Main Street Health possess these qualities:
- An entrepreneurial spirit. Must be a tenacious self-starter.
- Flexible and adaptable to a constantly changing workload.
- Must enjoy working in a fast-paced environment.
- A sense of humor and a down-to-earth nature.
