Job Summary: Under the direction and supervision of a Registered Nurse (RN) and the Home Care Coordinator, the Certified Home Health Aide / Certified Nursing Assistant (CHHA/CNA) provides direct personal care, restorative and supportive services to K-day PACE participants in their homes, at the PACE center, and in other community settings. The CHHA/CNA assists participants with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) as authorized in the participant’s interdisciplinary Plan of Care, observes and reports changes in condition, maintains a safe environment, and supports participants in remaining safely in the community. Qualifications and Requirements: Education/Certification/Licensure: - High school diploma or equivalent required.  - Current, active, and unrestricted California Certified Nursing Assistant (CNA) certificate and Certified Home Health Aide (CHHA) certificate issued by the CDPH Aide and Technician Certification Section, in good standing and verified through primary source prior to hire and at each renewal.  - Current Basic Life Support (BLS)/CPR certification (American Heart Association or American Red Cross) required.  - Valid California driver’s license - Proof of current auto insurance meeting state minimums Experience:  - Minimum of one (1) year of experience working with a frail or elderly population.  - Applicants with less than one year of experience who meet all other requirements must complete K-day PACE training on working with a frail or elderly population upon hire and before providing independent participant care - Home health or PACE experience is preferred. Medical Clearance: - Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact. Duties and Responsibilities: Direct Participant Care - Provide personal care and ADL/IADL assistance as authorized in the participant’s Plan of Care, in the home, at the PACE center, and in the community, including bathing, showers, bed baths, oral and denture care, hair care, shaving, nail care (per policy), dressing, toileting, incontinence care, and perineal care. - Complete safe transfers and ambulation utilizing Durable Medical Equipment (DME) as required, including gait belts, walkers, wheelchairs, and mechanical (Hoyer) lifts, using proper body mechanics and the number of staff required by policy. - Reposition participants, assist with range of motion exercises, and carry out restorative care and therapy home programs as instructed by the RN, PT, or OT. - Provide catheter care, empty urinary drainage bags, and empty or change established ostomy appliances as directed by the Plan of Care; does not insert or remove catheters. - Monitor and promote skin integrity; inspect skin during care and report redness, open areas, or other changes immediately. - Assist with meal preparation, set-up, and feeding, following prescribed diets, food textures, liquid consistencies, and aspiration precautions; measure and record food/fluid intake and output as directed. - Measure and record vital signs (temperature, pulse, respirations, blood pressure, pulse oximetry), height, and weight as assigned, and report values outside of ordered parameters to the RN immediately. - Provide medication reminders only as specified in the Plan of Care; does not administer, set up, or alter medications. - Perform light housekeeping, laundry (including soiled linens), dishwashing, and cleaning of bathrooms and commodes as authorized in the Plan of Care, maintaining a clean, safe, and sanitary environment. - Accompany or escort participants to medical and other appointments and assist on transportation vehicles when requested. - Create and participate in group and one-on-one activities at the PACE center as requested. Observation, Reporting, and Documentation - Observe and promptly report changes in the participant’s physical, mental, emotional, behavioral, or functional status, as well as the home environment and caregiver situation, to the RN, Home Care Coordinator, or designee. - Identify and report safety hazards in the home, such as tripping hazards, lack of working smoke detectors, unsafe oxygen use, pest infestations, or unsafe food storage. - Document all care provided, observations, participant refusals, and communications accurately, objectively, and timely in the electronic health record in accordance with K-day PACE policy. - Report participant falls, injuries, incidents, and unusual occurrences immediately and complete incident documentation per policy. - Recognize and report suspected elder or dependent adult abuse, neglect, self-neglect, abandonment, isolation, or financial abuse.
