LPN (LEVEL 3)

We are seeking a compassionate and experienced Level 3 Licensed Practical Nurse (LPN) with DDI experience to provide high-quality care in a residential or community-based setting. The ideal candidate will have hands-on experience with G-tube feedings and tracheostomy care and a strong commitment to supporting individuals with developmental disabilities. Responsibilities: - Provide skilled nursing care, including G-tube feedings and tracheostomy care - Monitor and document client health status and report changes as needed - Administer medications and treatments per physician orders - Collaborate with the healthcare and direct support teams to ensure safe, person-centered care - Ensure compliance with DDI and state regulatory requirements   Knowledge & Abilities - Strong clinical assessment skills and the ability to develop and implement care plans. - Excellent communication and interpersonal skills. - Proficiency in managing and administering medications. - Ability to work independently and as part of an interdisciplinary team. - Proficiency in electronic medical records (EMR) systems and Microsoft Office Suite. Requirements: - Active LPN license in MD - Proven experience with G-tube and trach care - Previous experience working with individuals with developmental disabilities (DDI) - Strong documentation and communication skills - Reliable, professional, and compassionate - Shift • Monday – Friday 9am – 5:00pm and flexibility to accommodate individual needs and schedule variations.
