Primary Care Social Work Case Manager

In this role, you will provide essential social work and case management support to patients with complex healthcare needs, helping them overcome psychosocial challenges and access appropriate care. Working within a multidisciplinary virtual care team, you will collaborate with physicians, nurses, care coordinators, and other professionals to deliver personalized, patient-centered support. You will conduct remote consultations, assess individual needs, and develop tailored care plans that address emotional well-being, social determinants of health, and long-term care considerations. You will also support patients' families and caregivers while helping them navigate healthcare services and available community resources.
