Home Health Occupational Therapist (OT) Full Time & Part Time | San Francisco and San Mateo County — Dedicated Territory Assigned We are currently looking for a compassionate, organized, and team-oriented individual to fill the role of Occupational Therapist (OT) position for the Full Time in SF area & Part Time in Peninsula area. You will be assigned to one dedicated territory — San Mateo County — so you can build familiarity with your patients and your area without being stretched across both. SF patients are spread across neighborhoods like the Sunset, Richmond, and Mission; Peninsula patients run from Daly City down through Redwood City. - SF Rideshare Support: If you take the SF territory, company-provided rideshare credits (Uber/Lyft) help cover travel to and from patient visits so you can focus on care, not logistics. - AI-Assisted Documentation: We use AI-assisted charting so you spend less time on notes and more time with patients. - Clinical Support: You are never alone. Our experienced case managers and clinical team are available in real-time to assist with complex situations or patient coordination. Compensation & Benefits - Base Salary: $125,000 – $150,000 per year (based on experience) - Performance Bonus: Up to 25% of base salary tied to quality metrics and clinical outcomes - Total Earning Potential: $156,250 – $187,500 per year - Health: Medical, dental, vision, life, and pet insurance - Retirement: 401(k) with employer matching - Perks: Paid time off, vehicle reimbursement options, and company-provided devices (iPad) Your Role - Provide skilled occupational therapy services focused on ADLs, adaptive equipment, and functional independence - Develop, implement, and manage individualized plans of care based on physician orders - Report changes in patient condition and collaborate with physicians, case managers, and the Director of Nursing - Maintain accurate, timely clinical records using AI-supported documentation tools Requirements - Current, active California Occupational Therapist license (required) - Minimum of 1 year of home health experience strongly preferred - Valid California driver's license - Strong organizational skills, clinical judgment, and a compassionate communication style Ready to Connect?
