RN Care Navigator/Case Manager

This position is listed on behalf of a partner company, which manages all applications and next steps. Our partner is looking for an RN Care Navigator / Case Manager based in the United States. We are seeking a compassionate and experienced Registered Nurse to join a specialized care management team focused on improving outcomes for patients living with chronic kidney disease and related conditions. In this fully remote role, you will coordinate patient-centered care, manage a diverse caseload, and help members navigate complex healthcare journeys. You will conduct health assessments, develop individualized care plans, provide clinical education, and coordinate services with physicians, healthcare professionals, and community resources. A key part of your mission will be empowering patients and their families to make informed decisions, improve self-management, and access appropriate treatment options, including dialysis and transplantation. Working within an interdisciplinary healthcare environment, you will help improve care quality, continuity, and cost-effectiveness.
