Registered Nurse (RN) – Hospice Southern Grace Hospice | North Dallas / Plano, TX Job Type: Full-Time Location: North Dallas / Plano and Surrounding Areas Experience Required: Previous Hospice Nursing Experience Position Overview Southern Grace Hospice is seeking a compassionate, dedicated, and experienced Registered Nurse (RN) to join our growing hospice team in the North Dallas/Plano area. The ideal candidate is passionate about providing exceptional end-of-life care, supporting patients and their families, and working collaboratively with an interdisciplinary team to ensure comfort, dignity, and quality of life. Previous hospice nursing experience is required for this position. Key Responsibilities - Provide skilled nursing care to hospice patients in their homes and other care settings. - Complete comprehensive nursing assessments and develop individualized plans of care. - Manage pain and symptoms to promote patient comfort and quality of life. - Collaborate with physicians, hospice aides, social workers, chaplains, and other interdisciplinary team members. - Educate patients and families regarding disease progression, medications, symptom management, and end-of-life care. - Maintain accurate, timely clinical documentation in accordance with hospice regulations and agency policies. - Participate in interdisciplinary group (IDG) meetings and care coordination. - Provide compassionate emotional support to patients and their loved ones. Qualifications - Active, unrestricted Texas RN license – Required - Previous hospice nursing experience – Required - Strong clinical assessment, critical thinking, and symptom management skills. - Knowledge of hospice philosophy, end-of-life care, and Medicare hospice regulations. - Excellent communication, organization, and time management skills. - Ability to work independently while maintaining strong collaboration with the hospice team. - Valid driver’s license, reliable transportation, and ability to travel throughout the assigned service area. - Current CPR Certification What We’re Looking For We are looking for a dependable, compassionate RN who understands the unique needs of hospice patients and families.
