Housing and Transportation Program Manager

As the Housing and Transportation Program Manager, you will manage initiatives that help members transition from institutional or higher-acuity settings to safe, accessible, and sustainable community living. You will coordinate housing and transportation resources, build partnerships with landlords, housing authorities, transportation providers, and other stakeholders, monitor contract requirements and project deliverables, and work with internal teams to address barriers, improve access, and support successful transitions. You will also provides day-to-day project oversight and coach team members to strengthen program knowledge and performance.  The Illinois Tailored Care Management Program helps individuals with complex health and social needs transition from nursing facilities and Specialized Mental Health Rehabilitation Facilities (SMHRFs) to community-based living. Through person-centered care management and coordinated support services, the program empowers members to live more independently and thrive in their communities.
