Community Engagement Program Manager

As the Community Engagement Program Manager, you will manage community engagement activities and partnerships that connect members to culturally responsive resources, social services, benefits, and community supports. Working with internal teams and external stakeholders, you will coordinate initiatives that support successful transitions to community-based settings, identify access barriers, incorporate member and stakeholder feedback into program improvements, and track progress toward contractual requirements and program goals.  The Illinois Tailored Care Management Program helps individuals with complex health and social needs transition from nursing facilities and Specialized Mental Health Rehabilitation Facilities (SMHRFs) to community-based living. Through person-centered care management and coordinated support services, the program empowers members to live more independently and thrive in their communities.
