Social Worker

This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Social Worker based in the United States. This fully remote role provides behavioral health and social support to members facing complex health and social needs. You’ll assess psychosocial circumstances, identify social determinants of health, and help remove barriers to care. The role combines telephonic outreach, supportive counseling, care coordination, and community resource navigation. You’ll work closely with an interdisciplinary clinical care team to support comprehensive member outcomes.
