RN Care Navigator/Case Manager

This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a RN Care Navigator/Case Manager based in United States. This fully remote role supports patients navigating complex kidney health needs across the continuum of care. As a Care Navigator, you will manage a diverse caseload, coordinate services, and help patients understand their treatment and care options. You will combine clinical expertise with patient advocacy to promote high-quality, cost-effective outcomes. The role involves collaborating closely with physicians, nurses, families, and other healthcare professionals to address barriers and achieve individualized goals. You will also support medication management, health assessments, care planning, and ongoing follow-up. Candidates must be able to work virtually across Pacific and Mountain time zones, with no travel required.
