Social Worker - Community team

Liquid Personnel is recruiting Social Worker for its client's Community team at Trafford. What will your responsibilities be? - Undertake assessments of adults’ care and support needs. - Develop, implement and review person-centred care and support plans. - Identify and respond to safeguarding concerns, including abuse, neglect and self-neglect. - Promote independence, wellbeing, choice and control. - Assess and manage risks while promoting individuals’ rights and autonomy. - Apply relevant legislation, including the Care Act 2014 and Mental Capacity Act 2005. - Work collaboratively with health professionals, families, carers and partner agencies. - Arrange and review care packages and community-based support. - Support hospital discharge, crisis intervention and prevention of admission where appropriate. - Provide information, advice, advocacy and access to community resources. - Undertake mental capacity assessments and best-interests decision-making where required. - Maintain accurate case records, reports and statutory documentation. - Participate in supervision, team meetings and ongoing professional development. - Ensure practice complies with safeguarding procedures, organisational policies and Social Work England standards.
