Position Summary
Paragon Behavioral Health Connections is seeking a Bachelor-Level Care Manager to provide intensive community-based care management services within the Community Access & Partnership (CAP) Program.
The CAP program serves adults with Serious Mental Illness (SMI) and Severe and Persistent Mental Illness (SPMI) whose psychiatric, functional, and social needs create significant barriers to engaging in traditional outpatient services. Utilizing assertive community outreach and a multidisciplinary team approach, the Care Manager provides intensive care coordination, resource navigation, housing support, community linkage, benefits assistance, and skill development services within clients' homes and natural environments.
The Care Manager plays a critical role in helping clients achieve psychiatric stability, maintain housing, reduce crisis utilization, improve independent living skills, and successfully transition toward less intensive levels of care.
This position requires extensive field-based work, community outreach, and collaboration with internal and external providers throughout the Denver Metro area and surrounding communities.
