Care Coordinator - CPST (Residential Facility) Location: Columbus, Ohio Job Type: Full-Time (In-person) Pay: $19.00 - $24.00 per hour Schedule: Sunday - Thursday | 11:00 AM - 7:00 PM Overview We are seeking a dedicated and compassionate Care Coordinator - CPST to support youth within our residential treatment program. This position serves as a primary link between youth, families, placing agencies, OhioRISE, community providers, and the treatment team to ensure coordination of services, discharge planning, and successful treatment outcomes. The Care Coordinator provides Community Psychiatric Supportive Treatment (CPST) services when medically necessary, maintains an assigned caseload, supports treatment and discharge planning, and ensures critical communication is maintained among all stakeholders involved in a youth's care. Key Responsibilities Caseload & CPST Responsibilities - Maintain an assigned caseload and remain knowledgeable of each youth's treatment goals, discharge needs, safety concerns, family supports, and barriers to progress.  - Provide CPST services to assigned youth when identified within the treatment plan and medically necessary.  - Assist youth with improving daily functioning, problem-solving, coping skills, symptom management, discharge readiness, and successful functioning at home, school, and in the community.  - Conduct CPST sessions with youth and collateral contacts, including family members, guardians, county workers, OhioRISE representatives, and community providers.  - Coordinate referrals and connections to community behavioral health, medical, educational, vocational, recreational, and natural supports.  - Monitor changes in youth functioning, behaviors, treatment needs, and barriers to progress while communicating significant concerns to the treatment team. Treatment Team & Family Communication - Participate in treatment team meetings, service plan reviews, county meetings, OhioRISE meetings, family meetings, court hearings, and other case conferences as assigned.  - Maintain regular communication with families, guardians, custodians, placing agencies, OhioRISE care coordinators, and approved supports while ensuring confidentiality requirements are met.  - Coordinate family involvement, family visits, and family communication when clinically appropriate.  - Prepare or contribute to monthly progress updates, county reports, court reports, and treatment team documentation. Discharge & Aftercare Planning - Begin discharge planning upon admission and continually address barriers to successful discharge throughout placement.  - Coordinate aftercare services, appointments, educational supports, community resources, medication follow-up, and treatment recommendations for successful reintegration into the community.  - Support QRTP aftercare requirements and maintain follow-up contact as assigned.  Appointments & Care Coordination - Coordinate medical, dental, vision, psychiatric, specialist, and community appointments with nursing, therapists, and administrative staff.  - Ensure appointment outcomes, recommendations, and follow-up needs are properly communicated and documented. Documentation & Safety - Complete CPST progress notes, case management notes, contact logs, discharge planning documentation, and related records accurately and within established timelines.  - Document interventions provided, youth responses, progress toward treatment goals, and next steps.  - Immediately report safety concerns, abuse or neglect allegations, suicidal ideation, self-harm concerns, threats, elopement risks, medication concerns, and other urgent clinical issues.  - Utilize trauma-informed care principles and de-escalation techniques during crises and difficult situations.  - Attend supervision, staff meetings, training activities, audits, and quality reviews as assigned. - QualificationsEducation - Bachelor's degree required in: - Social Work - Psychology - Counseling - Human Services - Sociology - Criminal Justice - Behavioral Health - Or a related field - Experience - Experience in behavioral health, case management, care coordination, CPST, community mental health, residential treatment, or closely related services preferred.  - Experience working with children and adolescents strongly preferred.  - Experience working with trauma, aggression, crisis behaviors, family systems, child welfare systems, or youth with problematic sexual behaviors preferred. - Knowledge & Skills - Knowledge of trauma-informed care and behavioral health treatment approaches.  - Strong case management, care coordination, and communication skills.  - Ability to collaborate effectively with families, community providers, county agencies, OhioRISE representatives, and multidisciplinary treatment teams.  - Strong documentation, organization, and time-management skills.  - Ability to remain calm and effective during crisis situations while utilizing de-escalation techniques. - Why Join Us - Make a meaningful impact in the lives of youth and families. - Work within a supportive and collaborative multidisciplinary treatment team. - Receive ongoing training, supervision, and professional development opportunities. - Gain valuable experience in residential behavioral health and CPST services. - Grow your career within a mission-driven behavioral health organization.
