LEAP Program Description
Global Solutions Ventures (GSV) – a joint-venture partnership between ZemiTek, LLC and Dexis Consulting Group – is implementing the Long-term Exceptional Technical Assistance Project (LEAP Global), a mechanism originally used by USAID to battle against infectious diseases such as malaria, HIV/AIDS, tuberculosis, neglected tropical diseases, and pandemic influenza. As of July 1, 2025, LEAP Global transitioned to the U.S. Department of State (DOS). This initiative aims to address the complex human resource challenges faced by national infectious disease programs in the USA-supported countries.
The intervention of this project is to hire and embed technical expert advisors and subject matter experts within government entities. By directly embedding the technical expertise within the national programs, LEAP provides long-term, sustainable lifesaving support to empower host governments to lead and manage key aspects of their infectious disease portfolios and improve coordination between the U.S. Government and the host country.
About the Position
The Senior Health Systems Advisor will provide strategic and technical assistance to the Department of Health (DOH) in the development and implementation of the One Hospital Command as a regional patient navigation and referral system connecting primary care with appropriate diagnostic, hospital, and specialty services across public and private providers.
The Advisor will support DOH in developing a common operating model that can respond to regional differences in health facilities, geography, workforce, financing, and service capacity. The position will support both urgent and planned referrals and strengthen coordination across levels of care to help ensure that patients are appropriately accepted, referred, provided needed services, and linked back to the originating provider for continued care.
The Advisor will also support the application of strengthened referral and patient navigation systems to priority public health needs, including tuberculosis (TB), particularly where improved linkages between primary care, diagnostic services, treatment facilities, hospitals, and specialty services can strengthen continuity of care.
The Advisor will provide technical expertise to design the regional care and referral model, establish necessary operating processes and agreements, assess financing pathways, develop options for a professionally managed command center, identify opportunities to activate priority facility capacity, and support pilot implementation.
The Advisor will work closely with the designated DOH Hospital Cluster lead, Centers for Health Development (CHDs), DOH hospitals, Local Government Units (LGUs), PhilHealth, participating private providers, and other relevant stakeholders. The position will provide technical advice and implementation support. Final decisions on policy, financing, staffing, procurement, contracting, and facility operations shall remain with the appropriate authorized government and institutional officials.
The Advisor will support the DOH in carrying out life-saving humanitarian assistance (LHA) service delivery in full alignment with the new Presidential Executive Order (EO) directives.
Responsibilities
The Senior Health Systems Advisor will support the conceptualization, operationalization, and pilot implementation of One Hospital Command as a regional referral and patient navigation system.
Key responsibilities include:
- Assess existing referral pathways, patient navigation arrangements, service capacity, demand, and gaps across proposed pilot regions.
- Develop a regional care and referral model defining roles, referral pathways, acceptance and escalation processes, feedback mechanisms, transfer arrangements, and back-referral processes across public and private providers.
- Support the development of regional network maps, service directories, referral criteria, standard operating procedures, and agreements with participating providers.
- Support the application of strengthened referral and patient navigation systems to priority health programs, including TB, to improve linkages across appropriate levels of care.
- Assess financing pathways and barriers affecting completion of referrals, including applicable PhilHealth benefits, LGU and other lawful financing arrangements, and costs associated with transport, diagnostics, medicines, and professional services.
- Develop and assess options for the One Hospital Command operating model, including in-house, outsourced, and hybrid arrangements for appropriate command center and coordination functions.
- Identify opportunities to activate priority facility and service capacity through targeted investments, shared services, private provider participation, management or service contracts, or public-private partnership options.
- Support selection, preparation, implementation, and monitoring of pilot regions and patient referral pathways.
- Develop recommendations and a phased approach for expansion of the model based on pilot implementation, lessons learned, costs, readiness requirements, and financing and staffing implications.
SPECIFIC RESPONSIBILITIES:
Regional Care and Referral Model
- Map existing referral pathways, demand, service gaps, patient travel patterns, provider capabilities, and patient navigation arrangements in proposed pilot regions.
- Differentiate appropriate pathways for cases suitable for primary care, direct specialty consultation, diagnostic referral, hospital admission, emergency transfer, and return to primary care.
- Define the roles of referring providers, receiving facilities, regional command centers, CHDs, DOH hospitals, LGU health systems, PhilHealth, and the national coordination function.
- Establish proposed acceptance, escalation, feedback, transfer, and back-referral rules, including pathways across public and private facilities and regional boundaries, where needed.
- Develop regional network maps, service directories, referral criteria, standard operating procedures, and proposed agreements with participating providers, using existing networks and platforms where feasible.
- Ensure that the design of regional referral pathways considers priority public health programs, including TB, where relevant to diagnostic, treatment, hospital, and specialty care referrals.
Network Financing
- Map applicable PhilHealth benefits, provider contracting and payment arrangements, approved Special Health Fund uses where available, and other lawful financing sources for priority patient pathways.
- Identify financing gaps that could prevent referrals from being accepted or completed, including costs related to transport, diagnostics, medicines, and professional services.
- Develop practical financing and agreement options with PhilHealth, LGUs, health boards, and participating facilities.
- Clarify proposed payment responsibilities, contracting or disbursement arrangements, required approvals, and approaches for tracking cases and payments, subject to applicable laws, regulations, and approval of the responsible authorities.
Command Center Operating Model
- Define the proposed operating model for a regional patient navigation and referral center, including hours of operation, clinical triage and escalation, nonclinical call handling, case ownership, quality assurance, workforce requirements, supervision, training, data protection, and continuity arrangements.
- Define interfaces between the command center, health facilities, patient navigation mechanisms, and emergency response systems.
- Compare in-house, outsourced, and hybrid models for appropriate call center and coordination functions.
- Develop recommendations including cost estimates, performance standards, clinical governance safeguards, data and system requirements, and draft specifications or terms for proposed service arrangements.
- Define a minimum information set and reporting process covering bed availability, operating service capacity, specialists and other personnel, equipment, diagnostics, critical supplies, and service interruptions.
- Link current capacity gaps with approved facility development plans and milestones to support appropriate assignment and monitoring of actions.
Priority Facility Capacity
- Identify facilities and services that could be made functional through targeted investments, shared services, private provider participation, management or service contracts, or public-private partnership options.
- Assess demand, readiness, staffing, operating costs, financing requirements, procurement routes, and time to service for identified priority opportunities.
- Develop a shortlist of potential investable or contractable opportunities and prepare decision briefs for DOH and relevant LGUs.
- Distinguish immediate operational improvements from projects requiring formal feasibility studies, procurement processes, or PPP approval.
- Coordinate with relevant technical, finance, legal, and procurement offices in developing recommendations.
Pilot Implementation, Monitoring, and Scale-up
- Support DOH in selecting pilot regions and referral pathways based on readiness, regional variation, and potential for learning.
- Support participating offices in establishing agreed workflows, preparing staff, developing baseline information, and launching pilot implementation.
- Facilitate regular case reviews and support rapid identification and correction of failed referral handoffs.
- Monitor referral completion, time to acceptance and service, failed and repeated referrals, patient follow-up, capacity data completeness, financing barriers, and service gaps.
- Track both operating performance and progress of actions intended to activate missing facility and service capacity.
- Document implementation lessons, costs, decisions required, and minimum readiness conditions for expansion.
- Develop a phased plan for extension to additional regions, including accountable offices, financing and staffing implications, and national-level support requirements.
