Patient Referral Coordinator — NuNavigator Program
Remote | Full-Time | NuNavigator Patient Services
Who We Are
NuvemRx helps community health providers deliver on their mission by simplifying the complexity of pharmacy operations, with a suite of scalable solutions that integrate software, intelligence, and expert support to illuminate savings, expand access, and elevate patient care.
What We Are Looking For
At NuvemRx, we hold a high bar because our work directly impacts patient care. We look for people who embody our core values:
- Mission Always: Every choice is a chance to serve our customers and improve healthcare
- Be a Pioneer: Take initiative, solve problems, and build what does not yet exist
- Do the Right Thing the Right Way: Lead with integrity, compliance, and thoughtful execution
- Stronger Together: Own your role, collaborate deeply, and elevate the team
We are also a fast-growing company, which means you bring strong ownership, comfort with ambiguity, resilience, and a bias toward action.
About the Role
The Patient Referral Coordinator keeps NuNavigator patients connected to the specialty care they need. Working alongside our FQHC partner, you manage the full referral-to-appointment journey: scheduling specialist visits, reminding patients so they show up prepared, managing no-shows and waitlists, and setting up follow-up care. You also help patients understand how the onsite pharmacy supports their care and keep information flowing smoothly between the health center and specialty offices.
This is a fully remote role, open to candidates based anywhere in the United States. We have a strong preference for candidates located in the Sharon Hill, PA (Greater Philadelphia) or Dallas, TX area, though the right person will be considered regardless of location.
What You’ll Do
Scheduling & Referral Coordination
- Schedule specialist appointments for referred patients and confirm details with the patient and specialty office.
- Deliver appointment reminders via phone, text, and other channels to reduce missed visits.
- Manage no-shows and cancellations — follow up with patients, reschedule promptly, and keep the process moving.
- Maintain and work waitlists, filling earlier openings quickly as they become available.
- Schedule follow-up appointments and confirm patients complete their recommended care.
Patient Education & Care Coordination
- Educate patients on the benefits of using the FQHC’s onsite pharmacy, including care coordination, convenience, and improved access to medications.
- Coordinate with specialty care offices to ensure referral documentation, records, and results are shared accurately and on time.
- Keep referral and scheduling records current and accurate in the patient management system.
- Handle all patient information in accordance with HIPAA and NuvemRx compliance policies.
What You Bring
Core Capabilities
- 2+ years in a healthcare, medical office, pharmacy, or patient services setting; scheduling, referral coordination, or intake experience preferred.
- Strong phone and written communication skills with a patient, service-oriented manner.
- Comfort with EHR, scheduling, or referral management software and general computer proficiency.
- Ability to manage a high volume of appointments and referrals while keeping records accurate and organized.
- High school diploma or equivalent.
Preferred Experience
- Experience in an FQHC, community health, or care coordination environment.
- Familiarity with specialty care referral workflows.
- Bilingual in English and Spanish.
Who Thrives in This Role
- You are meticulous about follow-through — nothing falls through the cracks on your watch.
- You bring genuine empathy and professionalism with patients who may be navigating a stressful or difficult health situation.
- You are comfortable juggling competing priorities, switching between systems, and coordinating across multiple offices.
- You collaborate naturally with health center teams, specialty offices, and pharmacy staff to keep patients moving through their care.
- You are adaptable and curious, especially as technology continues to shape how care coordination work gets done.
