Participant Care Associate, CNA

Position: Participant Care Associate (CNA) Client: PACE at Hudson Headwaters Location: Glens Falls, NY Target Areas: Queensbury, South Glens Falls, Hudson Falls, Fort Edward, Lake George, Saratoga Springs, Ballston Spa, Wilton, Moreau, Gansevoort, Corinth, Warrensburg, Granville, Whitehall, Schuylerville, Greenwich, Argyle, Salem Pay: $22.50/hour Schedule: Full-Time; Monday–Friday; 8:00 AM - 4:30 PM; No weekends required About the Role: Provide direct personal care and support to older adults enrolled in a Program of All-Inclusive Care for the Elderly (PACE), helping maintain safety, dignity, independence, and quality of life while working as part of an interdisciplinary healthcare team. Key Responsibilities: • Provide hands-on personal care (bathing, grooming, dressing, toileting) • Assist with feeding and meal support • Assist with transfers and ambulation • Monitor participant well-being and report concerns • Complete participant care documentation • Support participant independence and safety • Follow individualized care plans and collaborate with interdisciplinary team members • Maintain participant dignity and comfort Required Qualifications: • Active New York State CNA Certification • High School Diploma or GED • Ability to provide hands-on personal care and work with elderly and frail populations • Experience or comfort assisting with bathing, grooming, dressing, toileting, feeding, transfers, ambulation • Ability to work Monday–Friday, 8:00 AM - 4:30 PM • Ability to lift up to 50 lbs occasionally • Reliable transportation • Ability to complete medical clearance and background checks • Strong communication and documentation skills • Ability to work within an interdisciplinary healthcare team Benefits: • Full-time employment • Monday–Friday schedule (no weekends) • Paid Time Off structure with six paid holidays included • Employer-paid BLS certification • Paid orientation and training with structured competency-based training • Interdisciplinary team support #HireFinder #LI-PROMOTED #LI-Onsite
