Service Coordinator

We are seeking an experienced Service Coordinator to support participants in the Nursing Home Transition and Diversion (NHTD) Waiver program. This role is ideal for someone with hands-on NHTD experience who is compassionate, organized, and comfortable managing multiple cases independently. Responsibilities - Coordinate services and care for NHTD Waiver participants. - Develop, review, and update service plans based on participants’ needs. - Communicate with participants, families, caregivers, and service providers to support continuity of care. - Monitor service delivery, follow up on outstanding needs, and address coordination issues. - Maintain accurate case records and documentation in accordance with program requirements. - Manage a caseload independently while maintaining timely communication and follow-up.
