Improving healthcare through innovative technology is at the core of Intelerad’s work. Our scalable medical imaging platform connects clinicians to a powerful imaging ecosystem that is fast, smart, and tapped into the data they need, no matter their location. We’re focused on delivering a best-in-class medical image management solution that improves provider efficiency, decreases the cost of healthcare, and improves the overall health of populations.   Intelerad offers one of healthcare’s most comprehensive imaging and analytics platforms. Headquartered in Raleigh, NC and Montreal, Intelerad has nearly 700 employees located in offices across six countries. The company empowers nearly 2,000 healthcare organizations around the world with the speed, scalability, and simplicity needed to increase business performance while, most importantly, improving patient outcomes. Intelerad’s modern enterprise solutions have been acknowledged by a Best in KLAS recognition, ranking #1 for PACS Asia/Oceania in the 2021 Best in KLAS: Global Software (Non-US) report. Role Overview When radiologists receive the right study at the right time, patients get answers faster, and behind every worklist, search, and assignment is a database that has to respond quickly and correctly. As a Senior Software Developer focused on the data layer of our InteleOrchestrator team, you’ll modernize the SQL Server-backed platform that streamlines radiology workflows for healthcare providers around the world. You’ll write the code that changes the database: refactoring schemas and queries, untangling legacy data-access code, and moving it to type-safe, well-tested patterns while the system stays live for clinicians. This is a hands-on software development role, not a database administration role; your time will be spent in code, query plans, and pull requests rather than in backups, patching, or server operations. Success in this role means faster, safer, and more maintainable data access that reduces clinical burden and gives time back to the clinicians who depend on it. Key Responsibilities - Lead complex database refactoring across a large, mature SQL Server schema, including restructuring tables, keys, and indexes, retiring legacy structures, and backfilling and normalizing data, delivered as safe, incremental, version-controlled migrations (e.g., Liquibase) with no disruption to clinical users. - Diagnose and resolve performance problems at the source by reading execution plans, rewriting queries to be sargable and set-based, replacing row-by-row processing with batched writes and properly indexed temp tables, and designing covering and filtered indexes based on real workload evidence. - Modernize the data-access layer by migrating legacy and inline SQL in PHP and C# code into type-safe Kotlin and jOOQ repositories, preserving behavior through characterization, regression, and scale testing. - Harden data access for security and reliability by eliminating SQL injection through parameterization and automated checks, establishing consistent locking, isolation, and query-timeout conventions, and building query attribution and diagnostics that trace database load back to the code responsible for it. - Build the tooling that makes database change routine, including migration and upgrade pipelines in Gradle and CI/CD, containerized SQL Server test environments, and automated checks that catch regressions before release. - Shape technical direction by defining SQL and data-access standards, documenting conventions, and contributing to architecture decisions and the InteleOrchestrator technical roadmap. - Partner with and mentor fellow developers through code reviews, knowledge sharing, and technical guidance, helping application teams write efficient, safe database code and fostering a culture of continuous learning. Qualifications & Experience - Bachelor’s degree in Computer Science, Engineering, or a related field, or equivalent experience. - 7+ years of professional software development experience, including 4+ years in which writing and refactoring database code (SQL, schemas, and data-access layers) for production systems was a core part of the role. - Expert-level T-SQL on Microsoft SQL Server, including query optimization, execution plan analysis, indexing strategy, temp tables, CTEs, window functions, stored procedures, and set-based design; PostgreSQL experience is also valued. - Proven track record of incrementally refactoring a large, legacy relational schema and the application code that depends on it while in production, not only greenfield database design. - Proficiency in Kotlin or Java, or an equivalent language, building backend services with Spring Boot or an equivalent framework. - Hands-on experience with database migration and version-control tools such as Liquibase or Flyway, treating schema as code within Git-based CI/CD pipelines. - Strong understanding of concurrency, locking, isolation levels, and transactional integrity, and how they affect application behavior under load. - Solid grounding in secure data-access practices, including SQL injection prevention and safe handling of sensitive patient data. - Experience delivering in agile teams using Gradle, Git, and modern DevOps practices across Windows and Linux environments.Preferred Qualifications & Special Requirements - Experience with jOOQ or similar type-safe SQL libraries and code generation from database schemas. - Experience reading, maintaining, and porting legacy application code in PHP and/or C# (.NET). - Experience with database testing approaches such as Testcontainers or Docker-based SQL Server environments, golden-master or characterization tests, and load or scale testing of queries. - Familiarity with SQL Server diagnostic tooling (Query Store, DMVs, Extended Events) and observability platforms such as Datadog or New Relic. - Experience with AWS data services such as Amazon RDS for SQL Server. - Experience working with healthcare data standards such as HL7 and DICOM, or building software in other regulated industries (e.g., FDA- or FAA-regulated environments). - Location: This role is open to candidates based in the United States or Canada. This job description is not a comprehensive list of tasks or requirements and Intelerad reserves the right to change it at any time.
