At Revere Health, we believe there is a better path to healing and healthcare maintenance, and we’re working on this mission—one patient at a time. We’re a national leader in a movement called value-base care which aims to improve treatment outcomes and keep costs down. Our internal culture is one that promotes respect and consistently recognizes the impact that individual employees have on the mission of the organization. Position Summary: As the Orthopedic Patient Transition Coordinator, you will play a key role in supporting patients during the transition of our orthopedic clinic. This position focuses on providing compassionate, knowledgeable, and efficient assistance to help patients navigate their next steps in care. You will guide patients in identifying appropriate providers within our organization or in the broader community, ensuring continuity of care and a positive patient experience. You will also assist the Orthopedic Administrator with inventory management and other office transition tasks. Essential Duties and Responsibilities: Serve as the primary office contact for patients seeking orthopedic follow-up or continued care; Assist patients in understanding their care options, including in-network and external resources; Guiding patients with complex questions or inquiries regarding their care and follow up options; Work closely with internal departments and clinics to ensure smooth handoffs; Maintain up-to-date knowledge of available providers and services in the region; Ensure timely communication with patients via phone, email, or other communication channels; Document all interactions in the electronic health record (EHR); Address and escalate patient concerns or complex cases to leadership or care coordination teams as appropriate; Maintain confidentiality and comply with HIPAA and other relevant regulations; Take inventory of all assets, DME, Medications, and other medical supplies. Assist with closure of outstanding vendor accounts; Assist with other office transition tasks as necessary and assigned. May be required to travel to other Revere Health clinics within Utah County as needed. Qualifications:  Knowledge of orthopedic terminology, procedures, and common conditions (obtained through experience in an orthopedic or healthcare setting); Excellent interpersonal and communication skills, with a patient-centered approach; Strong organizational skills and attention to detail; Proficiency in using electronic health record systems (e.g., TouchWorks, Epic, Cerner) and Microsoft Office Suite; Ability to work independently and collaboratively in a fast-paced, transitional environment; Prior experience in referral coordination, patient navigation, or clinical support roles; Experience in working with insurance plans and understanding in-network vs. out-of-network referrals; Medical Assistant or similar clinical background (preferred but not required). This is a temporary position established to support patients during the transition period. We anticipate the position to end on November 30th, 2026.
