Community Liaison / Resource Navigator

As a Community Liaison / Resource Navigator, you will connect individuals with community resources and programs as needed and provide non-clinical support to care managers. You will develop and maintain relationships with community resources, facilities, state agencies, and community entities, and work collaboratively with healthcare professionals and social service agencies to provide members with supports and resources that will eliminate barriers, overcome challenges, and improve access to housing opportunities.     The Illinois Tailored Care Management Program helps individuals with complex health and social needs transition from nursing facilities and Specialized Mental Health Rehabilitation Facilities (SMHRFs) to community-based living. Through person-centered care management and coordinated support services, the program empowers members to live more independently and thrive in their communities.
