American Primary Healthcare is seeking an experienced, strategic, and people-focused Director of Nursing & Clinical Operations to provide leadership across our skilled home health nursing, private duty nursing, therapy, and DODD service lines. KEY LEADERSHIP RESPONSIBILITIES 1. Skilled Home Health Nursing - Provide clinical oversight and support for nursing services. - Monitor clinical documentation, assessment, care coordination, patient outcomes, and quality measures. - Support compliance with applicable Medicare, Medicaid, Ohio, and payer requirements. - Promote appropriate clinical practices, patient safety, and survey readiness. 2. Private Duty Nursing - Support nursing staffing, competency, supervision, and service delivery. - Monitor continuity of care, documentation, and adherence to individual care plans. - Address clinical concerns and promote consistent, high-quality nursing services. 3. Therapy Services - Coordinate with therapy leadership and qualified therapy professionals to support service quality and interdisciplinary communication. - Monitor documentation, care coordination, and compliance with applicable professional and payer requirements. - Help identify opportunities to improve service delivery and patient outcomes. 4. DODD Services - Support clinical coordination and collaboration across applicable DODD service programs. - Promote compliance with relevant Ohio DODD requirements, individual service plans, safety procedures, and incident-reporting processes. - Partner with appropriate program leadership to strengthen staff training, documentation, quality, and individual-centered care. 5. Organizational Leadership - Recruit, develop, mentor, and retain qualified clinical personnel. - Establish clear performance expectations and accountability. - Lead quality assurance and performance improvement initiatives. - Support audits, inspections, corrective action plans, and continuous readiness. - Collaborate with the Administrator and leadership team on staffing, operational efficiency, service quality, and strategic growth. - Identify opportunities to improve communication, workflows, employee engagement, and organizational performance. REQUIRED QUALIFICATIONS - Current, unrestricted Registered Nurse (RN) license applicable to Ohio. - Demonstrated nursing leadership, supervisory, or clinical management experience. - Experience in home health and/or private duty nursing preferred. - Familiarity with Medicare, Medicaid, clinical documentation, quality improvement, and regulatory compliance. - Strong communication, organizational, problem-solving, and team-development skills. - Ability to manage multiple priorities, develop staff, and work collaboratively with multidisciplinary teams. Experience with therapy operations and Ohio DODD services is highly desirable.
