CDS Case Manager Location: Joplin, Missouri Job Type: Full-time, on-site Compensation: $18.00–$20.00 per hour Make a meaningful difference every day BAYADA is seeking a compassionate, organized CDS Case Manager to support consumers and their attendants throughout the Joplin service area. This is a full-time, nonclinical role responsible for coordinating Consumer Directed Services (CDS), maintaining accurate documentation, supporting compliance, and helping consumers receive dependable in-home care. The CDS Case Manager will manage a caseload of consumers and work with attendants who provide care for those consumers. The ideal candidate is dependable, comfortable prioritizing multiple responsibilities, and committed to treating every consumer and family with dignity and respect. Service area and work environment - Primary location: Joplin, Missouri - Coverage area: Jasper, Newton, McDonald, Lawrence, Barry, Vernon, Cedar, Dade, and Barton counties - Work model: 100% on-site at the Joplin office - Schedule: Monday through Friday, 8:30 a.m. to 5:00 p.m. - Travel: Weekly field visits are required, including visits to new admissions and annual face-to-face visits. Some rural locations may be up to two hours away. - A fleet vehicle will be available for business travel when possible. Mileage may be reimbursed when a fleet vehicle is unavailable and a personal vehicle is used. - No on-call requirement What you’ll do - Complete admission paperwork and verify submitted information for accuracy, completeness, and compliance. - Enter consumer care plan changes and authorizations with the state. - Conduct monthly case management calls and complete required weekly field visits, including new-admission and annual face-to-face visits. - Coordinate documentation and follow-up for a caseload of consumers. - Support attendants who work for consumers receiving CDS services. - Monitor compliance requirements and maintain accurate, timely case records. - Process weekly payroll errors and exceptions for CDS employees. - Process attendant onboarding paperwork and help ensure required information is complete. - Order supplies for consumers as needed. - Help attendants troubleshoot their electronic visit verification (EVV) application. - Perform related office and administrative duties to support smooth daily operations. - Build positive relationships with consumers, attendants, families, and community partners. - Participate in outreach and marketing activities to help grow the caseload. Important: This is a nonclinical coordination role. Consumer assessments and reassessments are completed by the state. What you’ll need Required - High school diploma or equivalent experience. - Strong organizational and prioritization skills. - Ability to manage multiple deadlines, documents, and follow-up activities accurately. - Ability to work on-site and travel throughout the assigned service area. - Comfort working in a variety of home and community environments. - Professional communication skills and a respectful, service-focused approach. Preferred - Previous CDS, home care, human services, healthcare administration, or related experience. - Experience working with electronic documentation, payroll, scheduling, or electronic visit verification systems. What success looks like During the first 90 days, you will learn BAYADA’s CDS processes, build relationships with consumers and attendants, and work toward independently managing your caseload. Success in this role requires attention to detail, follow-through, empathy, and the ability to remain organized while responding to changing needs.
