Liquid Personnel is recruiting an Adult’s Social Worker to join its client’s Advice and Wellbeing Hub team, based in Reading on a six-month contract. This is an excellent opportunity for a skilled practitioner to support the reduction of waiting lists whilst delivering timely, strengths-based interventions to adults, carers, and families.
The Advice and Wellbeing Hub acts as the first point of contact for adults, carers, and professionals seeking advice, support, and direction. Working within a fast-paced environment, you will manage a complex caseload, undertake robust assessments, manage risk, and support individuals with a wide range of needs including older adults, people with disabilities, substance misuse, homelessness, hoarding, and carers requiring support and guidance.
Using a person-centred and strengths-based approach, you will help individuals build resilience, maximise independence, and access community resources that promote positive outcomes and wellbeing. The service works in line with outcome-focused practice and places strong emphasis on professional judgement, prevention, and early intervention.
What will your responsibilities be?
- Acting as a first point of contact for adults, carers, and professionals seeking advice, support, and guidance.
- Managing a varied and complex caseload involving adults with diverse and multifaceted needs.
- Undertaking strengths-based Care Act 2014 assessments, reviews, and support planning to identify eligible needs and desired outcomes.
- Completing timely risk assessments and making defensible professional decisions to promote safety and wellbeing.
- Developing person-centred Care and Support Plans that maximise independence, choice, control, and resilience.
- Supporting adults with complex needs including physical disabilities, long-term health conditions, substance misuse issues, homelessness, hoarding behaviours, and social isolation.
- Undertaking safeguarding enquiries and responding appropriately to safeguarding concerns in accordance with Adult Safeguarding legislation.
- Completing Mental Capacity Assessments and contributing to Best Interest decision-making in line with the Mental Capacity Act 2005.
- Working collaboratively with health professionals, housing providers, voluntary organisations, community services, carers, and partner agencies to coordinate effective support.
- Providing information, advice, signposting, and access to preventative services that help individuals build long-term resilience and independence.
- Maintaining accurate, timely, and legally compliant records, assessments, reports, and case notes using electronic recording systems.
- Contributing to service improvement initiatives and supporting high-quality, outcome-focused practice across Adult Social Care.
Qualifications and Experience:
To be successful in this role you must have;
- Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, or equivalent recognised Social Work qualification.
- Registration: Must be registered with Social Work England.
- Experience: Significant post-qualified experience within Adult Social Care services.
- Assessment Skills: Proven ability to undertake complex assessments, reviews, support planning, and risk management interventions.
- Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Mental Capacity (Amendment) Act 2019, Mental Health Act 1983, and Adult Safeguarding legislation.
- Safeguarding Experience: Experience undertaking safeguarding enquiries and supporting adults at risk.
- Multi-Agency Working: Experience working collaboratively with health services, housing providers, voluntary organisations, care providers, carers, and partner agencies.
- Communication Skills: Excellent communication, analytical, report-writing, negotiation, and decision-making skills.
- Professional Practice: Commitment to strengths-based, person-centred practice and continuous professional development.
- Organisational Skills: Ability to manage competing priorities, work independently, and perform effectively within a busy and demanding environment.
