Remembrance, Resolution, Reconciliation, Restoration, Renewal About the Position: As a Care Coordinator, you provide support and guidance to students while enrolled at Circle Tree Ranch. They facilitate access to resources, coordinate services, and ensure individualized transition planning. Responsibilities include conducting assessments, monitoring treatment plans, advocating for student needs, and maintaining accurate documentation. Care Coordinators collaborate with clinical staff, demonstrators, and external agencies to promote holistic well-being. Strong communication, crisis intervention, and problem-solving skills are essential. Full-Time This is a Non-Exempt position (Hourly). On-site
