Our client based in Leicestershire is seeking an experienced Social Worker to join their Cognitive and Physical Disability Team. This is an excellent opportunity for a dedicated practitioner to support adults with learning disabilities, physical disabilities, neurological conditions, and complex care and support needs through strengths-based, person-centred social work practice.
The successful candidate will work closely with individuals, carers, families, health professionals, and partner agencies to promote independence, wellbeing, and positive outcomes. You will undertake statutory assessments, risk management, safeguarding interventions, and care planning, ensuring people are supported to remain in their communities and exercise choice and control over their lives.
This role requires a confident Social Worker who can manage complex casework, work collaboratively across multidisciplinary teams, and apply relevant legislation to support vulnerable adults in achieving their desired outcomes.
What will your responsibilities be?
In this role, you will:
- Managing a varied caseload of adults with cognitive impairments, physical disabilities, neurological conditions, and complex support needs.
- Undertaking Care Act 2014 assessments, reviews, reassessments, and support planning to identify eligible needs and promote positive outcomes.
- Developing, implementing, and reviewing person-centred Care and Support Plans that maximise independence, wellbeing, and community participation.
- Acting as Care Coordinator for individuals with complex needs and coordinating support across multiple agencies and services.
- Undertaking Mental Capacity Assessments and facilitating Best Interest decision-making in accordance with the Mental Capacity Act 2005.
- Identifying, investigating, and managing safeguarding concerns, ensuring adults are protected from abuse, neglect, and exploitation.
- Assessing and managing risk appropriately whilst promoting positive risk-taking and individual choice.
- Supporting individuals, carers, and families to access direct payments, community resources, preventative services, and specialist support.
- Working collaboratively with health services, housing providers, community organisations, voluntary services, and partner agencies.
- Facilitating complex care and support arrangements and supporting individuals through periods of crisis, change, or deterioration in health.
- Preparing professional reports, assessments, court documentation, and funding applications where required.
- Maintaining accurate, timely, and legally compliant records using electronic case management systems.
- Participating in multidisciplinary meetings, safeguarding discussions, reviews, and case conferences.
- Contributing to service development, quality improvement initiatives, and the sharing of knowledge and expertise across Adult Social Care.
Qualifications & Experience:
To be successful in this role you must have:
- Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, or equivalent recognised Social Work qualification.
- Registration: Must be registered with Social Work England.
- Experience: Significant post-qualified experience within Adult Social Care services.
- Disability Experience: Experience supporting adults with physical disabilities, learning disabilities, cognitive impairments, neurological conditions, and complex care needs.
- Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, mental health legislation, and associated statutory guidance.
- Assessment Skills: Proven ability to undertake complex assessments, reviews, support planning, and risk management interventions.
- Safeguarding Experience: Experience undertaking safeguarding enquiries and managing high-risk situations.
- Multi-Agency Working: Experience working collaboratively with health professionals, carers, providers, voluntary organisations, and partner agencies.
- Communication Skills: Excellent communication, analytical, report-writing, negotiation, and decision-making skills.
- Professional Practice: Commitment to strengths-based, person-centred practice and continuous professional development.
- Organisational Skills: Ability to manage complex workloads, prioritise effectively, and work both independently and as part of a multidisciplinary team.
